As parent or guardian, I give _________________________________________permission to attend the Area Wide Crusade to be held at Wilson Park in Maynardville, TN from about 9:00 - 11:00 am on:
                                 May 9, 10, & 11 for the high school and alternative school students.
                                 May 11, 12, & 13 for middle and elementary school students.

            I understand chaperones will be provided at all times while my child is away from school.  The chaperones will include volunteers from area churches, parents, and high school students from the FCA.  (Fellowship Christian Athletics)

            Permission slips are due back by Thursday, May 5th for all schools.  Permission slips will be accepted anytime, but it will help us to better prepared and organized if you return them early.
